Prescription Forms - Where to Endorse

PSD has noticed a decrease in the quality of data being captured by their scanners (ICR software).  You could help greatly by ensuring that you only use the designated areas on the prescription form for your endorsements.  Careless endorsing can obliterate other important information which is then hard for PSD to capture. This slows up the process and increases the risk of PSD not meeting its deadlines for processing payments. Prescription Form Example

To view information on the Courier Service for Prescription Submissions for Pharmacy Contractors, please click here.

* For single collections please adhere to the date in this column.

** Some of the dates above are Saturdays. If you do not open on a Saturday please have your submission ready for the following Monday.
(Most of the Saturday collections will be in Ayrshire and Arran)

Posted submissions should be sent to: Practitioner Services Pharmacy, 3 Bain Square, Kirkton Campus, Livingston EH54 7DQ.

Further supplies of stationery forms should be ordered through your Board contact. You should make sure you know the time needed to order and receive additional supplies of stationery from your Board in order to ensure you do not run out of forms before you receive your next supply.
