Prescription Forms - Where to Endorse

PSD has noticed a decrease in the quality of data being captured by their scanners (ICR software).  You could help greatly by ensuring that you only use the designated areas on the prescription form for your endorsements.  Careless endorsing can obliterate other important information which is then hard for PSD to capture.  This slows up the process and increases the risk of PSD not meeting its deadlines for processing payments.

Here is a list of the contacts responsible for stationery in your Health Board area where supplies of other Pharmacy Forms and Stationery are available from the stores departments e.g.
                        CP2, A4 and CP1 forms             (Minor Ailments)

                        CPUS                                       (Urgent Supply form)

Further supplies of stationery forms should be ordered through your Board contact. You should make sure you know the time needed to order and receive additional supplies of stationery from your Board in order to ensure you do not run out of forms before you receive your next supply.
