CMS Implementation Payments (CIPS)

During 2010-11 it was agreed that the Chronic Medication Service should commence with an implementation phase which will run to 31 December 2010 (see PCA (P) 2010 No 10.) Under this plan contractors have been asked to:

  1. Identify all the patients with long term conditions who they believe will benefit clinically from CMS

  2. Register no more than 50 patients at the premises

  3. Provide CMS to these patients during this phase

The progress of this phase will be overseen by an independent review group in the first few months. A full decision on how roll-out will proceed after December will be taken later this year.

CMS will be supported during 2010/11 with a series of CMS implementation payments (CIPs). The payments must be claimed by submitting the appropriate form to PSD (see below).


Implementation Payment A (CIP-A) – standard rate of £750 per contractor

This payment is available to claim at any time before 31 December 2010 provided:

  • The contractor intends to provide CMS to no more than 50 patients until 31 December

  • All systems need to support CMS are in place, i.e. appropriate IT is in place, staff have been trained and appropriate SOPs have been developed

  • The pharmacy is ready to use the Pharmacy Care Record to conduct pharmaceutical assessments and develop where necessary pharmaceutical care plans

  • The pharmacist has established a provisional list of patients who are expected to benefit clinically from CMS and out of this list the first 50 have been identified for the service


Implementation Payment B (CIP-B) – standard rate of £750 per contractor

This payment has been made available for contractors to claim between 1 October 2010 and 31 December 2010. In order to qualify contractors must:

  • Continue to meet the criteria for CIP - A

  • Have started to deliver CMS to no more than 50 patients


Implementation Payment C (CIP-C) - variable amounts

This payment is intended to underpin the activity needed as contractors start to provide CMS for more than 50 patients. It will be made in 2 parts and will include both a fixed and a variable amount. Initially contractors will be paid an aspiration payment, linked to a notional registration figure which has been derived using previous dispensing activity for the period April-September 2010. Once actual registration figures are known an adjustment will be paid.

Notes

  1. The part 1 aspiration payment will be the amount due for the band below that in which the contractor sits

  2. The amount for the final settlement is dependent upon the registration band achieved

  3. If the contractor does not achieve the aspiration figure no further payment is made

  4. If the contractor reaches the aspiration figure set for him he will be paid the difference between the amount already received and the actual payment for the band in which he sits

  5. If the contractor exceeds the aspiration figure set, he moves up the bands and again gets the difference between the figure already paid and the appropriate band amount


Example

Contractor A dispenses on average 3700 items between April and September.

  • He/she receives notification in January that he/she has been allocated to Band 3 and a notional registration figure of 151-200 has been set

  • He/she paid £500 + £1500 (aspiration) in April with payment for his/hers February prescriptions

  • He/she sends in his claim form at the beginning of April and declares that he/she is providing CMS to 125 patients at the end of March 2011

  • He receives a final settlement in May 2011 of £250 only because he did not achieve a notional registration figure of 151+.
