Smoking Cessation Service
Contractors in the Fife area should note that the national minimum dataset form for smoking cessation has now been updated for use in Fife.

Contractors should ensure all sections of the form are completed as appropriate and submitted in a timeously fashion to the smoking cessation administrators in the Board.

Contractors should note the forms should be sent on a geographical basis to three different locations in Fife.

Dunfermline and West Fife:
Smoking Cessation Administrator, House 2, Lynebank Hospital, Dunfermline KY11 4UW

Kircaldy and Levenmouth:
Smoking Cessation Administrator, Randolph Wemyss Memorial Hospital, Buckhaven. KY8 1HU

Glenrothes and North East Fife:
Smoking Cessation Administrator, Ladybank Clinic, Commercial Road, Ladybank KY15 7JS
